About This Survey

Our medical office is participating in a survey for the U.S. Agency for Healthcare Research and Quality (AHRQ) that focuses on patient safety and workplace safety for providers and staff in medical offices. Westat, a survey research organization under contract with AHRQ, is conducting this web survey and analyzing the results.

  • There are no direct benefits to you for participating in this study. Your responses will help AHRQ with research focusing on patient safety and workplace safety for providers and staff.
  • Your participation in this survey is voluntary. You can skip any question and can discontinue the survey at any time.
  • There are no foreseeable risks to participating in this study. If you decide not to participate, it will not affect your job.
  • The information you provide will be kept confidential. Your individual responses will not be shared with AHRQ, our medical office, or any external organization. Comments provided at the end of the survey will be de-identified and may be provided in an overall report to all participating facilities.
  • The survey will take about 20 minutes to complete.
